The below link is only to be used by GP Surgeries. If you are a patient, you do not need to complete any forms from Oakdale before you visit your GP. You can find more information about our assessments HERE

When referring to Oakdale under NHS Right to Choose, please ensure the client is removed from any other waiting lists for the same assessment.

Once you submit the form, a copy of the completed form will be attached to an email sent to the address you provide.

Completed forms must be sent to:

ndassessments.referrals@oakdalecentre.org in order for us to proceed with the referral.
